We have some forms that we need completed before your student joins us for this production- now you can do it online before the first day. The only item we will need you to complete in person is a Student Info Card which will only take 3-5 minutes to complete.
Please make sure to READ AND SIGN our Orientation packet which will answer frequently asked questions. Click the link below to complete the packet. This must be done on or before the FIRST DAY. After you register, in the confirmation page or email there will be a link to sign the documents online.
For Volunteer Deposits-You can turn in a post dated check (Dated 5/12/2020). If you would prefer to use A Credit Card for your Volunteer Deposit, complete the form below. We will not run your credit card unless you do not complete/sign up for the required hours by May 12, 2020.

YOU MUST BE SIGNED UP FOR THE JOB YOU SELECTED ON SIGN UP IN ORDER TO GET CREDIT. You will not receive credit for any volunteer work not signed up for online. YOU ARE RESPONSIBLE FOR SIGNING IN AND OUT THE DAY YOU VOLUNTEER WITH STARBOUND STAFF. If you are not signed in, you will not receive credit.NOTE: No children are allowed during volunteer hours unless prior arrangements are made with Starbound Staff.
Families with more than one student must complete 8 hours. 8 hours is the max for families with more than one student in the program. Volunteer Deposit is $120.
I am choosing to opt out instead of fulfilling the volunteer requirement. I understand I must submit a payment for the required amount of $75/$120.

Participation in Fundraisers are OPTIONAL. The cost of producing theatre is expensive. We supplement the fees each production by holding a fundraiser. We’re asking each family to participate in the fundraiser and help the production become the best experience possible for our students. I have read the following and understand my responsibilities1. ITEMS TO TURN IN ON THE FIRST DAY- Student Info Contact Card and Deposit of $75 (Check or Credit Card Authorization Form) are also due the FIRST DAY!Forms are available to download above.2. Students should bring a snacks & bottled water to every rehearsal.3. CELL PHONES- Students must check in cell phones on arrival. Students may not film any other student and post any pics/videos from rehearsal on social media. Students are welcome to take pictures/videos once in their parent's care. No use of cell phones is allowed backstage by students AND parents. Students will pick up cell phones before dismissal.4. NO GUM, SODA, FLIP FLOPS/SANDALS, 5. Students need to bring 3 ring binder and pencil daily. Students will not be able to take scripts home until binder is brought in.6. Prior to the first rehearsal, a withdraw form must be completed and emailed to the office PRIOR to the first day if requesting a partial refund. (Refund would be 1/2 of the registration fee minus a 10% admin fee,) Once registration is confirmed, my place is held and fees cannot be refunded unless I purchase the protection offered below.In order to get a FULL refund, you must purchase the Protection below. I understand that Starbound Theatre is not affiliated with the Purchase Protection plan and I will have to work with Regfox on any claim.
