I am aware that participation in the Starbound theatre program involves an element of risk and danger of accidents. I am participating and/or allowing Participant(s) to participate in this activity with knowledge of the danger involved. I accept and assume all risks of injury, death, and property damage. I agree to release, waive, discharge and convenant not to sue Starbound Theatre, Rodgers Enterprises, David Rodgers Jr, Bliss Urban Arts Center, its officers, agents, volunteers or employees, and any associated agents, staff and volunteers from all actions, claims, demands or liability for any bodily injury, death or property damage arising out of or in any way connected with my and/or Participant(s) participation in this activity, including any provided transportation services. I also grant full permission to Starbound Theatre, Rodgers Enterprises, David Rodgers Jr to use my and/or Participant(s) name and/or photograph, videotape, motion picture or recording for any publicity or promotion purposes without obligation, compensation or liability.

BY CHECKING THE BOX, I UNDERSTAND THIS RELEASE AND WAIVER OF LIABILITY, I AGREE AND ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THE INFORMATION. I UNDERSTAND I AM VOLUNTARILY GIVING UP LEGAL RIGHTS AND THAT THIS IS A LEGAL, BINDING AND ENFORCEABLE CONTRACT. I UNDERSTAND THERE ARE NO REFUNDS. I UNDERSTAND THERE ARE NO REFUNDS ON REGISTRATION FEE AFTER THE FIRST DAY. I UNDERSTAND THAT THERE IS A NON-REFUNDABLE DEPOSIT TO HOLD MY SPOT IN THE PRODUCTION. I UNDERSTAND THAT PROGRAMS MAY IMPACTED BY GLOBAL EVENTS AND ACTS OF GOD AND THAT PRODUCTIONS MAY BE POSTPONED, CANCELLED AND/OR CHANGED. I UNDERSTAND THERE IS NO REFUND OR CREDIT OFFERED IF THIS SHOULD HAPPEN. I AM FULLY AWARE OF MY RESPONSIBILITY TO PARTICIPATE IN FUNDRAISERS AND THAT I AM LIABLE FOR ANY MATERIAL GOODS USED FOR FUNDRAISING PURPOSES.
